GUIDE Program
The GUIDE Program is personalized dementia care for you in your home to provide additional support and improve quality of life. All led by compassionate local Peterson Providers.
Supportive coordinated care, in your home, led by your nurse care navigator to help improve the quality of life for people with dementia, reduce burden and stress on family caregivers, and to prevent or delay long term nursing home care.
GUIDE care includes:
- Comprehensive in home assessment
- Ongoing support for family, friends, and caregivers
- Respite and relief services for eligible caregivers
- Personalized care plan
- Education and skills training for family, friends, and caregivers
- Medication management
- Care Coordination and referrals to community resources
- No out of pocket cost for patients
Eligibility Criteria
- Traditional Medicare Part A and Part B Only
- Hospice patients and long term care facility residents are not eligible per Medicare guidelines
- Dementia or suspected dementia
