Health Talks: Support and Planning During a Hospital Stay

This Health Talk explains how hospital case managers help patients and families during a hospital stay. Participants will learn how case managers coordinate care, help explain treatments and medications, plan for a safe discharge, and connect patients to services after leaving the hospital.

Examples of questions to ask will include: Who is my case manager? How can they help me? What support will I need when I go home? What services or resources are available to me? The goal is to help patients and caregivers feel supported, informed, and prepared.
