That is why Peterson Health is proud to offer support through the GUIDE Program; a care model focused on improving the quality of life for patients living with dementia while also supporting the caregivers who care for them every day. 

As the number of Americans living with Alzheimer’s disease and related dementias continues to rise, programs like GUIDE are helping families receive more personalized, coordinated care close to home. 

What is the GUIDE Program? 

The Guiding an Improved Dementia Experience (GUIDE) Model is an eight-year nationwide program created by CMS to improve dementia care through a more comprehensive and patient-centered approach. 

Through Peterson Health’s GUIDE Program, patients receive support from an interdisciplinary care team that works together to create individualized care plans based on each patient’s goals, strengths, and needs. These plans help coordinate care between providers, specialists, and community resources while also addressing co-existing health conditions. 

The program is designed to help patients remain safely at home for as long as possible while improving communication, care coordination, and overall support for families navigating dementia. 

Supporting Caregivers Every Step of the Way 

Caring for a loved one with dementia can be overwhelming, both emotionally and physically. One of the most important parts of the GUIDE Program is caregiver support. Peterson Health’s care team works closely with caregivers by providing education, ongoing monitoring, support services, and access to helpful community resources. 

The program also includes 24/7 access to support services, helping caregivers feel less alone during difficult moments and giving them tools to better care for their loved ones.
